How do I reset my password on Provider Portal?

You can reset your password from the log in page of the Provider Portal.

  1. Click on "Forget Password"

  2. Enter your email address

  3. Follow the instructions sent in the email.

Do note that your password must contain a minimum of 8 characters.

You will also be locked out from your account for an hour after 5 unsuccessful login attempts. However, you can unlock your account immediately by resetting your password with the aforementioned steps.
